Screening Questions
You should keep the questions below in mind before every colour appointment, it is your responsability to inform me of any reason you may need a new allergy alert test
Before every colour appointment, you will be emailed a form directly from my booking system called Fresha containing the following information and questions.
This message is sent out both when booking the appointment and 48 hours before with your appointment reminder email however, the questions must be completed a minimum of 3 days and a maximum of 5 days before your colour service, If you answer “YES” to any of the following questions, a new allergy alert test must be carried out a minimum of 48 hours before your colour service. If any of your answers change between completing this form and your appointment, you must inform me.
Please note if this is your first colour appointment with me or you are returning to have colour for the first time after reopening 12/4/21 you will need an allergy alert test regardless of the screening questions.
You will be asked to create an account with a user name and password that you can log in with and keep track of your consultation forms
Please do not ignore the email, it may result in your appointment being cancelled and you still being charged.
If you do not receive an email please check your junk folder and contact me if not so I can update your email address.
You will be asked the following questions if you answer YES to any of them you will be required to have a new Allergy Alert Test
Has it been 12 months or more since your last allergy alert test?
Have you had an allergic reaction to and products or hair colour since your last professional colour service was carried out?
Are you pregnant?
Have you had any type of skin tattoo, including semi-permanent, makeup or black henna?
Have you got an irritated or damaged scalp?
Have you had any serious illnesses since your last colour service? (this includes Covid-19)
